Diazepam Drug Uses, Dosage & Side Effects

Benzodiazepine Side Effects

The benzodiazepine hypnotics shorten the time it takes to fall asleep and prolong the sleeping period. The main differences exist in how long they might remain in the body, possibly leading to prolonged side effects. For example, lorazepam has a much shorter duration than diazepam, allowing quicker clearance of the drug and theoretically less side effects. On the other hand, diazepam can remain in the system for days and boost the risk for long-term side effects, especially in the elderly. Problems start to arise when benzodiazepines are taken at higher dosages than recommended, or when they are taken for more than two to four weeks.

There was an association between the prescription of anxiolytic drugs and mortality, resulting in 4 excess deaths in the anxiolytic drug group within an average of 7.6 years [47]. The relationship between hypnotics and cancer was expanded upon by Kripke et al. They found a 35% increased chance of developing a new non-melanoma cancer in users of hypnotics [48]. In agreeing with some of the studies above, there was also a 4.6-fold increase in the hazard of death in patients on hypnotics over 2.5 years [48].

Understanding Benzodiazepines and How They Work

Occasional anomalies (reduction of tarsals, tibia, metatarsals, malrotated limbs, gastroschisis, malformed skull, and microphthalmia) were seen in drug-treated rabbits without relationship to dosage. Although all of these anomalies were not present in the concurrent control group, they have been reported to occur randomly in historical controls. At doses of 40 mg/kg and higher, there was evidence of fetal resorption and increased fetal loss in rabbits which was not seen at lower doses. Tolerance is a physiological state characterized by a reduced response to a drug after repeated administration (i.e., a higher dose of a drug is required to produce the same effect that was once obtained at a lower dose). Tolerance to the therapeutic effect of Ativan may develop; however, little tolerance develops to the amnestic reactions and other cognitive impairments caused by benzodiazepines. Though it helps many people, this medication may sometimes cause addiction.

Many drug therapies have been suggested as treatment for alprazolam withdrawal with few rendered effective. Captodiamine is a diphenhydramine-related compound that does not work at histamine receptors as diphenhydramine does and its mechanism of action is unclear [70]. This drug has also been studied in the context of both BZD replacement and withdrawal as a potential treatment [70]. One study showed that replacing BZD with a 45 day captodiamine led to a decrease in severity of withdrawal symptoms in patients taking BZD for six months [70]. Another interesting finding was that after the discontinuation of captodamine treatment, there was no emergence of withdrawal symptoms, suggesting that captodiamine might have a different mechanism of anxiolysis than BZD [70].

  • Panic disorder can be a prolonged, chronic disorder, but it is very treatable with medications that lessen symptoms.
  • In 2018, between 8.3% and 12.8% of BZD users in Switzerland have prescriptions from multiple physicians which resulted in the inability to track the number of prescriptions a patient is given yearly [40].
  • Ativan (lorazepam) belongs to a class of medications called benzodiazepines.
  • Most adverse reactions to benzodiazepines, including CNS effects and respiratory depression, are dose dependent, with more severe effects occurring with high doses.
  • You may have life-threatening withdrawal symptoms if you stop using the medicine suddenly after long-term use.

Related to their rapid onset and immediate symptom relief, BZDs are used for those struggling with sleep, anxiety, spasticity due to CNS pathology, muscle relaxation, and epilepsy. One of the debilitating side effects of BZDs is their addictive potential. The dependence on BZDs generally leads to withdrawal symptoms, requiring careful tapering of the medication when prescribed. Regular use of BZDs has been shown to cause severe, harmful psychological and physical dependence, leading to withdrawal symptoms similar to that of alcohol withdrawal. The current treatment for withdrawal is through tapering with clonazepam.

  • For most types of acute or prolonged seizures or status epilepticus, an intravenous (IV) or rectal benzodiazepine would be the treatment of first choice.
  • Some side effects of lorazepam may occur that usually do not need medical attention.
  • Captodiamine is showing promise as a potential medication for the management of BZD withdrawal syndrome; however, more research needs to be performed on the side effects and safety profile of the drug.
  • However, one study showed no significant difference in BZD use in MMT patients between gabapentin and placebo [68].

Benzodiazepine drugs (also called benzos) are habit forming and can lead to addiction. Long-term use also can lead to tolerance, which means that lower doses will become ineffective and patients will need higher doses. These drugs are abused to get ‘high’ due to their effects on the brain. Drug interactions may change severe benzodiazepine withdrawal syndrome how your medications work or increase your risk for serious side effects. Keep a list of all the products you use (including prescription/nonprescription drugs and herbal products) and share it with your doctor and pharmacist. Do not start, stop, or change the dosage of any medicines without your doctor’s approval.

Important considerations for taking diazepam

When they’re taken for longer periods, benzodiazepines can lead to tolerance and dependence, meaning that your body will require more of the drug to achieve the same therapeutic effect. Long-term use of benzodiazepines may trigger a worsening of side effects and, in some cases, paradoxical side effects, meaning that you may experience an opposite response to the drug than you previously had. Some of these side effects can adversely affect your mood and behavior, causing an altered perception of yourself, your environment, or your relationships. If you develop any unusual and strange thoughts or behavior while you are taking lorazepam be sure to discuss it with your doctor. Some changes that have occurred in people taking this medicine are like those seen in people who drink alcohol and then act in a manner that is not normal.

Only short-term use of low-dose benzodiazepines (2 to 6 weeks) is recommended, with a slow, gradual tapering once the antidepressant effect takes hold. Concerns about risks of tolerance, dependence, and diversion with benzodiazepines limit their usefulness in GAD. Benzodiazepines should be avoided in patients with a history of drug abuse.

What is the most important information I should know about benzodiazepines?

Your healthcare provider can answer questions about whether to change what you eat or drink while taking these medications. Fatal side effects can occur if you use this medicine with opioid medicine, alcohol, or other drugs that cause drowsiness or slow your breathing. General information about the safe and effective use of ATIVAN Medicines are sometimes prescribed for purposes other than those listed in a Medication Guide.
